Wheelchair Consultations.

PT evaluation and assessment for new and currently used wheelchairs are available including written reports, communication with physicians, fitting, and transfer training. PT on the Go will take you through the entire process and follow-up needs. We work closely with local medical suppliers and will connect you with the right person to meet your needs. You may be eligible for a new or upgraded wheelchair or features every 5 years or with a change in condition or mobility status.

Scooter and Power Wheelchair Services

$390 plus Mobile Service fee for each visit.

  1. Consultation and Evaluation with a Physical Therapist and the Mobility Specialist. 1 visit

2. Followup Fitting and Training with the Physical Therapist x 2 visits.

Payment for Wheelchair Evaluation, Reports, Fitting, and Training is based on private cash pay (or accepted insurance if you are a current PT patient on a plan of care.) Requirement of 3 hours service time includes Evaluation with PT and Mobility device company representative and Mobility Training x 2 visits when the wheelchair or scooter arrives.

Please note, the complete process requires full participation of your provider and receipt of referring provided signatures. Without the necessary signatures, the process will not be completed.
